File Upload Form provides a ready-to-use contact form with a built-in file upload field. Place the form anywhere with the [file_upload_form] shortcode. Visitors fill in their name, email, subject, message, and optionally attach a file. On submission the plugin validates the file server-side, attaches it to a notification email sent to the admin inbox, then deletes the file from the server so no binaries are stored permanently. Every submission (without the file binary) is recorded in a lightweight database table viewable in the WordPress admin.
