Provides a "Request a Callback" form via the [callback_form] shortcode. Visitors enter their name, phone number, preferred callback date and time, and optional notes. Submissions are emailed to the admin and stored in a custom database table. The admin can view all requests, update their status (Pending / Called / Cancelled), bulk-mark requests as called, and export the full list as a CSV file.
